You can create questionnaires and submit them to the other community members to solicit responses. Users can respond once they have been opened.

Like all other items, Questionnaires can be arranged into folders and can be viewed, edited, deleted or moved. They can also be given access-permissions to specify who can view and modify them.

Creating a Questionnaire

A questionnaire is created with a name and description. You then choose how users can respond to the questionnaire - eg. is it just available to members, or to anyone? can you respond more than once? are answers anonymous? does a user get a 'Thank you' email afterwards? You can also choose if and how responses are made available whilst the questionnaire is open and closed.

Once your questionnaire has been created, you must then add questions; a combination of either multiple choice, open or file. As well as questions, you can also add text/images which can be placed at any point amongst the questions. This is optional but can be used to enhance the questionnaires by providing helpful text for users who respond.

If some of your questions have similar question layouts, or use similar options, you can use the copy question icon to create a new question based on an existing one.

You can use page breaks to split a questionnaire into multiple pages. This can make the questionnaire less intimidating as fewer questions are presented to the user at a time. You can arrange your questions into pages after the questions have been added by clicking on the Re-order the questions action link. This lets you move questions and page breaks about until you have the right arrangement.

After all questions have been added, the questionnaire can then be opened which then allows community members to access and complete the questionnaire.

Questionnaires can have a pre-set close date, and the results can be made available to all members, or just to the creator. Private questionnaires can be created whereby the community members cannot see the results until the questionnaire has been closed, thus preventing the possibility of members being swayed by existing entries.

Once a questionnaire has been closed no further responses can be submitted but you can modify the questions then re-open it.

Question Details

With open questions, you have the option of specifying an input mask that can be used to constrain the answer that a person provides. In an input mask, special characters correspond to the possible values that a user can enter. These are:

nAn optional digit (0-9)
NA mandatory digit (0-9)
aAn optional letter (a-z or A-Z)
AA mandatory letter (a-z or A-Z)
?An optional character (any printable character)
&A mandatory character (any printable character)
\The escape character. This removes the special meaning of the mask characters

Any other character matches the character itself. The match is not case sensitive. Examples:

If you use specify an input mask, it is recommended that you specify the acceptable formats (ie. what is considered valid / invalid) in the question text, to assist people in answering the question.

By default, questions are asked in the order they are listed on the Manage Questionnaire page. You can include routing in your questionnaire, that takes a different path through the questionnaire based on a person's answers. For example, you can route in order to skip certain questions if a person answers "no" to a question rather than "yes".


Each type of question supports a different set of conditional tests that can be used for routing. You can add as many routing conditions as you like and they are evaluated in order. After evaluating all of the routing conditions, if none of them are met, then the questionnaire continues on to the next question. If a conditional test cannot be evaluated, for example because the question being tested was skipped, it will evaluate to "false".

Other Information

One or more items (eg. files, pictures etc.) may also be associated with an individual questionnaire, if related items are enabled for that questionnaire. This is done by adding a new Related Item to the questionnaire. These related items are then also visible to other community members, when they view that questionnaire, in the "Related Items" section.
